Your comfort and quality care is our primary concern.
If you are experiencing a dental emergency of any type we will do our best to see you the same day that you call. We will make every effort to take care of your concern immediately. After you are comfortable we will discuss and arrange appropriate appointments for ongoing care and maintenance.
Please assist us by providing the following information at the time of your consultation:
- X-rays if applicable or the name and phone number of your prior dentist so that they may be obtained.
- A list of medications you are presently taking
If you have dental insurance, bring your current insurance card. If you don’t have one, we will need the subscriber number, group number, and phone number. This will save time and allow us to help you in the processing of your claim.
IMPORTANT: All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your previous dentist has taken X-rays, please have them forwarded to our office as they may be helpful in our diagnosis. If you would like us to contact your previous dentist to request your films we will be happy to do so. Any digital radiographs can be emailed to us at drbob@monmouth.com. If additional films are necessary, they can be taken at our office.
